Approval from the local governing body (City or County Clerk's office) is required before entry of the portal application. The required forms are found below. Please keep in mind that the local governing body may require additional information or paperwork.
ENTRY OF APPLICATION:
Make sure you are selecting the right option for your application type. Non-Profit applicants, Retail applicants or Retail applicants with catering endorsements have different fees. There are NO REFUNDS for incorrect application types.

APPLICATION REMINDERS:
- The fee is $40.00 per date for each pecial Designated License Application, unless the applicant is the holder of a liquor catering license (K).
- Retail liquor license holders are allowed six (6) calendar dates per year (January - December).
- Retail liquor license holders that hold a catering license are allowed unlimited.
- There is a limit of twelve SDL license dates that can be obtained per location each calendar year.

This count includes all SDL licenses issued by all applicants at the location.

Only six consecutive dates can be entered on one application.

Alternate dates and locations must be approved by the local governing body and submitted with the original application.

There are NO refunds for late, rejected or canceled applications.

Application fees cannot be transferred to rescheduled events.

The issued license is available to download in your customer account after the local governing body has confirmed you have paid all fees and taxes to them.

The license must be printed and displayed at the event.
NON-PROFIT ORGANIZATIONS

Non-profit applicants can apply for up to six non-consecutive dates at the same location on one application.

The payment for the first date remains the same at $40.00, however the 2nd through the 6th date will be $10.00 per day.
- Non-Profit Affidavit - This form must be signed in front of a notary to register your non-profit organization with the commission
- Non-profit Registration Instructions - You must complete your non-profit registration before applying for your first SDL to get your Parent License number.
SDL Farmer's Market Endorsement

The Special Designated License Farmers Market Endorsement Application allows a Craft Brewery, Micro Distillery, or a Farm Winery to attend one Farmer's Market at the same location with one issued SDL that covers the entire retail license year.

The licensee will only be allowed to serve the type of alcohol produced by the applying license.

Each licensees that hold more than one license will need to apply for another license if they wish to sell other produced product (Class L, Class Y, Class Z) would have to each apply for the Farmer’s Market SDL.
